City Attorney Performance Evaluation Questionnaire
Instructions:
The response to each question provides a linear scale ranging from 1 to 5 between two opposing responses. The lower the number, the closer the respondent agrees with the response on the left. The higher the number, the closer the respondent agrees with the response on the right. A response of "0" indicates that the respondent is declining to answer the question, for whatever reason.
