You are authorized to proceed with the following work in connection with Conway Road Improvement project.

RFQ No. 4                                                                                                        $ 36,030.00

Increase scope of work to include provision of all labor, equipment, and materials to install utility substructure (conduit, vault, splice boxes) per PG&E composite drawing and specifications from PG&E, PacBell, and AT&T and sewer connection.

 

                                                                                      TOTAL ADD:            $137,862.00

 

Increase in Contract Time: 60 calendar days

This change order constitutes full and complete compensation for all labor, equipment, materials, overhead, extended overhead and profit required to perform the changes described above in accordance with the applicable provisions of the contract documents. This change order is not effective until approved by Engineer.
