1.     2004-0742 – Cedar Crest Nursing Home [Applicant] Partners Sunnyvale LLC [Owner]: Application for a Use Permit on a 2.13-acre site to allow a 4,538 sq. ft. (17 bed) addition to Cedar Crest Nursing Home for a total of 116 beds and 32,984 square feet.  The property is located at 797 East Fremont Avenue in an R-0 (Low-Density Residential) Zoning District. 2.     2004-0755 - Application for a Variance from Sunnyvale Municipal Code (SMC) Section 19.34.030 on a 5,940 square-foot site to allow a 10.5-foot combined side-yard setback where 12 feet is required.  The property is located at 767 Lakebird Avenue in an R-0 (Low-Density Residential) Zoning District.  (APN: 110-20-047) KD

3.     2004-0775 – Friedrich Ventures, Inc. [Applicant] Varni Properties [Owner]:  Application for a Special Development Permit on a 3.2-acre site to allow an indoor children’s recreation facility.  The property is located at 1165 North Fair Oaks Avenue in a C-2/PD (Highway Business/Planned Development) Zoning District.  (APN: 110-29-040) SL

4.     2004-0776 - Application for a Use Permit on a 6,578 square-foot site to allow a twelve-foot high accessory utility building (playhouse) in the front yard.  The property is located at 450 East Arbor Avenue in an R-0 (Low-Density Residential) Zoning District.  (APN: 204-14-029) AM
