Wednesday, October 27, 2004

2004-0742 – Cedar Crest Nursing Home [Applicant] Partners Sunnyvale LLC [Owner]: Application for a Use Permit on a 2.13-acre site to allow a 4,538 sq. ft. (17 bed) addition to Cedar Crest Nursing Home for a total of 116 beds and 32,984 square feet.  The property is located at 797 East Fremont Avenue in an R-0 (Low-Density Residential) Zoning District. (APN: 211-25-014) DO

In attendance: Darrell Folks, Applicant; Tracie Murray; Gerri Caruso, Administrative Hearing Officer; Diana O’Dell, Project Planner; and, Gloria Barron, Recording Secretary. 

Ms. Gerri Caruso, Administrative Hearing Officer, on behalf of the Director of Community Development, explained the format that would be observed during the public hearing.

Ms. Caruso announced the subject application.

Diana O’Dell, Project Planner, presented the staff report.  She summarized the subject application and recommended approval of the Use Permit subject to the Conditions of Approval recommended by staff. 

Ms. Caruso opened the public hearing.

Darrell Folks, Applicant Representative, stated that he received and reviewed a copy of the staff report.  He did not have any concerns or questions regarding the Conditions of Approval. 

Ms. Caruso closed the public hearing.

Ms. Caruso approved the Use Permit with the Findings provided by staff with the Conditions of Approval recommended by staff.

Ms. Caruso stated that the decision is final unless appealed during the 15-day appeal period.

The meeting was adjourned at 2:05p.m.
