When reporting a complaint, make sure you do the following:

• Provide a brief description of the violation or complaint

• Provide the exact location of the problem. Provide the house number and street name. If there is no address, then provide as much information regarding the location (i.e., corner of Mathilda Avenue and Olive Avenue)

• Provide your name, address, and telephone number. This information is confidential and will only be used by us in order to obtain additional information or to advise you of the status of your complaint. All complainants remain anonymous.
