Field Reservation Procedure

1) Obtain field application from Sunnyvale Community Center by calling 730-7721or print application.

2) Completed applications should be faxed or sent to the below address:

3) Applications will be considered on a first come, first serve basis. Applications received before reservation start date will be considered after all other applications received. Mail-In applications will be accepted on a second priority basis.

4) Incomplete applications will not be considered.

5) A copy of proof of residency must be attached to application. Acceptable forms of identification: drivers license, utility or phone bill.

6) To reserve on behalf of a company or organization, your (Sunnyvale) company or organization must provide the City with a signed letter of authorization on official letterhead, granting your representative to act as a delegate.

7) There is a 2-hour minimum for field reservations; Field Rental Application must be submitted at least ten business days before first day requested. Field reservation coordinator will respond to any request within two business days.

8)  If all requests can be accommodated, your credit card will be charged the appropriate fee and your reservation permit and receipt will be mailed to you.
