Advance Health Care
Directive
This service is available to assist you in filling
out the appropriate forms for an Advance
Health Care Directive which allows you to
choose an agent to carry out your wishes

BART Tickets
The Sunnyvale Senior Center office sells
BART tickets for adults 65 & older.

Blood Pressure Screening
Volunteer nurses provide free blood pressure
screenings each Monday and Thursday
from 10:00am-12:00pm in Health Services

Care (Case) Management
Care (Case) Manager Vivian I. Silva
provides care management services to
eligible clients through a pilot program
funded by the City of Sunnyvale and the
Council on Aging Silicon Valley. She
brings 14 years of care management
experience in Santa Clara County to the
Senior Center. Vivian will assist seniors
and their families in identifying care and
service needs. Sunnyvale residents, age
50+, are eligible for care management

need repairs in electrical, plumbing,
roofing, structural reinforcement, heating,
weatherization, kitchen or bathroom
work? Do you need a ramp, wheelchair
lift or grab rails installed? The following
programs may assist you if you live in
Sunnyvale:
• Low Interest Home Repair Loans
at 3% to 5% for 15 years, depending
on your family income.
• Loans & Grants for exterior painting
available.
• Home Access Grants of up to
$6,500 to disabled homeowners to
retrofit their homes for handicapped
accessibility.

Library Delivers Materials
for Homebound Readers
SOS (Special Outreach Services) provides
library materials to homebound readers.
Every month on a regularly scheduled day,
the SOS delivery person will deliver to
your door a bag of books and other media
of your choosing. SOS clients receive
materials in various formats, including all books, and books on tape) as well as media
like music CDs and non-feature videos.

Live Oak Adult Day
Services
Adult day care for seniors with Alzheimer’s,
Parkinson’s, stroke, depression, cancer, and
other long-term illnesses. Monday - Friday,
9am-3pm. Seniors enjoy pet therapy,
arts & crafts, seated exercise, breakfast,
hot lunch, lots of music and children!
Assistance in restroom. Day services only.
Not a residential facility. Caregivers—a
place for your loved one during the day.

Transportation Authority
(VTA) Senior/Disabled
Photo ID Card
A VTA representative will be at the
Senior Center from 10:00am-12:00pm
on 3/19 and 5/21 to take applications and
photographs for senior/disabled regional
transit connection discount cards. The
cost for this service is $3. You must be a
Medicare card holder and have valid photo
identification to qualify for a discount

available at the Senior Center front desk.

Let the legal and social work staff at
Council on Aging’s program, the Fair
Lending Project for Seniors, answer your
questions about refinance loans, reverse
mortgages, home improvement loans
and “foreclosure rescue” scams. Legal
representation, referrals to community
specialists and referrals to social services
are also available. All services provided
free-of-charge to Santa Clara County
residents age 60+, regardless of income.

Sunnyvale Community Services provides a
variety of services to over 12,000 disabled

them remain independent, and assists in

Located at 535 Old San Francisco Road in
Sunnyvale, this federally funded nutrition
site is open to those adults 60+. Donations

Santa Clara County offers a taxi/
accessible van program for residents who
cannot use fixed route buses or light rail
transportation. To determine eligibility

Wireless Hearing System
for Hearing Impaired
Thanks to the generosity of
Dr. Sandra Wendschlag of
Landmark Hearing Services, we recently
installed a special wireless system. You
will see a blue and white hearing symbol
like the one below to denote the presence
of the system. This system allows most
hearing aids to act as tiny loudspeakers
that deliver the sound from the P.A.
System (microphones, speakers, television,
etc.) directly into your ears.
If you have “telecoil” or “mic/telecoil”
mode on your hearing aid, switch it to
be connected to the sound system in the
room.
