Steps to Request a File Review:

  • Determine the address(es) for the file(s) that are being requested.
  • E-mail the request to Lori Hollingsworth at lhollingsworth@ci.sunnyvale.ca.us or fax the information on a cover sheet to (408) 730-7214.
  • File reviews are limited to ten files per appointment.
  • Every effort will be made to respond within ten business days as to the availability of the requested files, in accordance with Government Code Section 6250. If appropriate, an appointment time will be set.
  • Walk-in file reviews are not permitted.

Appointments:

Appointments are set up in two-hour time blocks:  mornings 8:00-10:00 or 9:00-11:00, or afternoons 1:00-3:00 or 2:00-4:00. The files will only be available for the scheduled two hours; if you arrive late and/or do not complete your review within the allotted time, you will need to schedule another appointment.

Photocopies:

  • Orders must be prepaid with a check or exact change at the time of your appointment.
  • Use of outside photocopiers is not allowed.
  • Requests for ten pages or less will be processed at the end of your appointment.
  • Requests for more than ten pages will be processed within five to ten working days and can be picked up or mailed.
  • Delivery service (Fed-X, UPS) is available when an account number is provided to us.
